Unlock Rules (Changed in Patch 7.26)

At the start, only Journey is available. Originally, higher tiers unlocked as you cleared content — the earlier guidance was "defeat at least two bosses" to open the ladder. Then Update 7.26 streamlined the climb:

Per the official Update 7.26 patch notes: "Clearing Hard difficulty now directly unlocks Nightmare, Hell, and Reality difficulty."

The efficient path for most players is therefore: Journey → Common → Hard clear → the top three open at once. No need to climb the ladder one rung at a time.

Hard is the unlock gateway

Because clearing Hard unlocks three tiers at once, your first serious "build a real loadout, beat the final boss" run should target Hard — the single clear is worth three unlocks, and Hard's one Battle Scar is learnable.

Chaos Battle Scars: Boon and Curse Rolled Together

Chaos Battle Scars are the difficulty ladder's signature mechanic, starting at Hard. Each scar carries a boon and a curse together — high risk, high reward. Named examples confirmed in the patch notes: Lover's Betrayal (shop-related), Silent Hermit (combat-adjacent, rebalanced in 8.5 so it no longer takes effect outside combat), and Speechless (a silence-type effect; 7.27 clarified that silence blocks Skills and Ultimates but not shooting or reloading).

The smart play, per launch guides: choose the curse your build can negate. Running huge regeneration or shield defense? Skip the healing-reduction curse. Running movement speed and range? The melee-damage curse may be irrelevant to you.
