No More Room in Hell 2 Infection Guide — Stages, Cures & How to Avoid It

Infection is the clock ticking under every No More Room in Hell 2 run. A zombie grapple can put a virus in your blood that — left untreated — kills your Responder permanently, and it changes your priorities instantly: objectives stop mattering, survival starts. This guide covers exactly how infection works, the stages you'll feel (and hear), the two treatments, and the habits that keep you out of the death spiral entirely.

A dark hallway — infection is everywhere in this game.
A dark hallway — infection is everywhere in this game.

Here's what you'll learn:

  • Every way infection enters your body (grapples, stamina, low health)
  • The infection stages, from silent incubation to the final veins
  • Phalanx Pills vs Gene Therapy — what actually cures you
  • Self-sacrifice and the "K button" as a last resort
  • Extraction as a cure, and how to prevent infection with skills

How You Get Infected

Infection was introduced in the December 2024 Infection Update (0.2.1) and refined through Early Access. The infection vectors, per the official notes and community testing:

  • Grapples. When a zombie grabs you, there's always a chance of infection — and that chance rises as your health drops. Fail the grapple minigame or get grabbed with no stamina and you're infected automatically.
  • Low health. Below roughly half health, any zombie attack can infect you, with the risk climbing the lower you go.
  • Shambler gas. Standing in a Shambler's death gas cloud gives "a very high chance of becoming infected" — and zombies that walked through the cloud hit harder infect-wise too.
  • Any damage, in 1.0's framing. The official Road to 1.0 video states it plainly: "Anytime a zombie deals damage or grabs you, you can become infected."
Empty stamina = a bite, not a fight

The most common infection on the ladder: sprinting and swinging until your stamina hits zero, then getting grabbed. With no stamina you can't break the grapple — the bite is automatic. Save a stamina reserve for shoves in crowds.

The Infection Stages

The virus plays out in stages (per the official 0.2.1 notes):

  1. Incubation. For a short period there are no signs at all. You're infected and don't know it — this is why team discipline matters.
  2. Onset. Signs begin: coughing is one of the first (teammates can hear it), plus "migraine aura"-style visual sensitivity. Over time the visual and auditory distortion grows.
  3. Severe symptoms. "In the later stages of infection your symptoms will grow more severe, and indicate that without treatment your time is running out." Expect heavy screen distortion and audio hallucinations — voices, noises that aren't there.
  4. Veins. In the final ~30–35 seconds before the timer ends, veins appear in the edges of your eyes. If you see the veins, you're out of time — pills take seconds, gene therapy takes longer than you have.

Untreated, the virus kills your Responder — and per the franchise's rules, your body rises as a zombie unless someone deals with it.

Per the official Infection Update 0.2.1 notes: "Once infected, your main focus in the game shifts from completing the map objectives to survival."

Phalanx Pills vs Gene Therapy

Treatment What it does Availability Notes
Phalanx Pills Resets infection progress — doesn't cure Semi-common, ~3 doses per bottle, ~5 minutes per dose Buy time to reach real treatment or extraction; can be taken at any stage
Gene Therapy Fully cures and grants immunity for the rest of the round Extremely rare, one-use Two-step: take a blood sample, wait ~30 seconds for the antivirus, inject. Dropping it resets the progress

The community wiki confirms each Phalanx pill bottle holds 3 doses. The gene therapy device is the only true cure in a match — but the 30-second wait makes it a risk: you need cover while it processes.

Pills are a delay, not a cure

Three doses ≈ 15 minutes of extra time. If you're deep in a map, pills get you to extraction — they don't save the Responder by themselves. Treat every pill as a countdown extension and move toward the exit or a gene therapy spawn.

Self-Sacrifice and Other Options

  • Self-sacrifice (the "K button") — introduced in 0.2.1 as a community request. You end your run on your own terms: it prevents you from turning into a zombie and has a positive effect on your team. It's also technically the third "treatment" for infection — an ending, not a cure.
  • Extraction while infected — if you make it out with the virus active, you're cured at CRC headquarters; your Responder starts the next match clean. This is the most reliable "cure" in the game.
  • Infection Resistance perk — a rare variant of the skill grants total immunity; the base version gives a large (roughly 50%) chance to resist infection. Rerolling skills is a real endgame Credits sink for exactly this reason.

How to Avoid Infection (Prevention Checklist)

  • Keep stamina above empty. Reserve 15–20% for shoves. No stamina = no escape = auto-bite.
  • Keep health high. Bandages and medkits aren't hoarding — every hit below half health is a lottery ticket for infection.
  • Kill Shamblers at range. They scream, they summon, and their corpse gas infects. See the weapons guide for the damage math on Shamblers (70 HP — one solid hit tier below a walker).
  • Take Sturdy Body, Thick Skin, or Infection Resistance on characters you care about — the class skills that keep health up or block infection outright.
  • Watch teammates. If someone coughs, they need cover to treatment — infection is a squad problem, not a solo one. The multiplayer guide covers comms etiquette for exactly this.

What to Do the Moment You're Infected

  1. Call it out — voice or ping. Your squad needs to know.
  2. Head for the nearest Phalanx spawn or gene therapy location you know of; loot containers along the way.
  3. If pills are all you find, take one and move toward extraction — don't keep pushing objectives.
  4. If the veins appear, take a pill immediately (a few seconds) or self-sacrifice — gene therapy won't finish in time.
  5. Extract. Alive and cured beats dead with a full inventory.

Infection is what turns a normal death into a permanent one — the full stakes are in the permadeath guide, and new players should practice the whole loop in Solo Mode, where infection can't cost you a character.

Frequently Asked Questions

How do you get infected in No More Room in Hell 2?

Zombie grapples (chance scales with low health; auto-infection with no stamina), any zombie damage below half health, and Shambler gas clouds. In 1.0's framing, any zombie damage or grab can infect you.

How do you cure infection in No More Room in Hell 2?

Gene Therapy fully cures (rare, two-step, ~30-second processing). Phalanx Pills reset progress without curing. Extracting while infected cures you back at CRC. Self-sacrifice ends the run before you turn.

What are the infection stages?

Incubation (no signs) → Onset (coughing, visual/audio distortion) → severe symptoms (heavy distortion, hallucinations) → Veins (final 30–35 seconds, out of time).

Do Phalanx Pills cure infection?

No — they reset the infection timer (~5 minutes per dose, 3 doses per bottle) so you can reach treatment or extraction. Only Gene Therapy cures in-match.

What happens if you extract while infected?

You're cured at CRC headquarters and your Responder starts the next match clean — extraction is the most reliable cure in the game.

How do I avoid getting infected?

Save stamina for shoves, keep health topped up, kill Shamblers before they scream, avoid their gas, and grab infection-resistance/health skills on characters you care about.
